Dear All,

we were invited by the Swedish Covenant Hospital to be a part of the Alliance for Health Equity Community Input Survey for Chicago and Suburban Cook County. The Alliance for Health Equity is a group of over 30 hospitals, local health departments and community organizations in Chicago and Suburban Cook County that are working together to conduct a Community Health Needs Assessment (CHNA). Our community’s input is very important and will help create a plan to improve community health. The survey is anonymous and should take about 5 minutes to complete. It is for respondents 18 and over, and is available in multiple languages (including Polish) at  http://bit.ly/AHECommunitySurvey. Although online submission is preferred, we also have paper copies available if anyone  prefers a paper survey.

Any completed paper surveys can be returned to the main reception. The survey is open until mid-January. We will post this information on our website for an easy access. Please share with your networks.
